After scheduling your first appointment, you will receive new client forms through our client portal system. For your convenience, we also have the forms available to download below.
If you are unable to fill out these forms at home, don't worry! We have extra sets of printed forms available in our lobby. Just arrive early to your first appointment to ensure you have enough time to fill out the required forms prior to meeting with your counselor.
